It provides helpful information for retirees and supports families and representatives in managing affairs following the death of a member. Please note: this guide is for general assistance only and is not a substitute for personal estate or family planning.<\/p>\n<p class=\"Default\"><strong>The Government of Canada Pension Centre<\/strong>\u00a0is the current administrator of the Plan commonly referred to as the \u201cPension.\u201d It is imperative that they be informed immediately upon the death of a member. They require an original death certificate, or the certificate issued by the funeral director showing the date of death.\u00a0<\/p>\n<p class=\"Default\"><strong>Note:\u00a0<\/strong><em>Obtain several original certificates from the funeral director because they may be required at other locations such as banks, insurance companies, and other agencies.\u00a0<\/em><\/p>\n<p class=\"Default\"><strong>For General Queries or Notification of a Death:\u00a0<\/strong><\/p>\n<p class=\"Default\">Public Works and Government Services Canada Government of Canada Pension Centre \u2013 Mail Facility. You can reach a pension expert by: Telephone Toll-Free: 1-800-561-7930 <span style=\"font-size:12.0pt;line-height:107%;\">Monday to Friday: 8:00 a.m. to 4:00 p.m. (your local time)<\/span><\/p>\n<p class=\"Default\"><strong>It is very important the Government of Canada Pension Centre be advised immediately upon the death of a member. Failure to do so could result in overpayments that must be repaid.\u00a0<\/strong><\/p>\n<p class=\"Default\">Within approximately two weeks of <strong>the Government of Canada Pension Centre\u00a0<\/strong>being notified of a death, a letter should be received from them advising the next of kin of benefits and options available to them. If, after four weeks, no advice has been received from <strong>the Government of Canada Pension Centre<\/strong>, it is recommended they be contacted by telephone for confirmation they have received the previous information. The survivor will be required to complete separate forms to establish other survivor benefits and insurance payouts if applicable.\u00a0<\/p>\n<p class=\"Default\">Other things to be considered by the Survivor are:\u00a0<\/p>\n<ul>\n<li>Taxing of the deceased person\u2019s Pension<\/li>\n<li>Continuation of Public Service Health Care Plan \u2013 Extended Health<\/li>\n<li>Continuation of the Pensioner\u2019s Dental Services Plan<\/li>\n<li>Direct Deposit (to be set up in the spouse\u2019s name)<\/li>\n<li>Canada Pension Plan lump sum death benefit<\/li>\n<li>Canada Pension Plan Survivor Benefits<\/li>\n<li>Advice to current or other previous employers\u00a0<\/li>\n<\/ul>\n<p class=\"Default\">Since <strong>pensions are payable only to the end of the month of death<\/strong>, any cheque not negotiated that cover a period after the month of death must be returned to the pension office. If payments are deposited directly to the pensioner\u2019s account, any payments made past the month of death will be recovered.\u00a0<\/p>\n<p><span style=\"font-size:12.0pt;line-height:107%;\">The important thing to remember is that, upon the death of a pensioner, the spouse will receive only fifty <strong>(50%)\u00a0<\/strong>percent of the pensioner\u2019s Superannuation. The Government of Canada Pension Centre upon contact will provide the necessary forms and will advise of the options available.\u00a0<\/span><\/p>\n<p class=\"Default\"><span style=\"font-size:11.5pt;\"><strong>Required Documents\u00a0<\/strong><\/span><\/p>\n<p class=\"Default\"><span style=\"font-size:11.5pt;\">The following documents and information are necessary to obtain these benefits:\u00a0<\/span><\/p>\n<ul>\n<li><span style=\"font-size:11.5pt;\">Pension number (can be found on the Annual Pensioners\u2019 Statement)<\/span><\/li>\n<li><span style=\"font-size:11.5pt;\">Date of death (death certificate is required in each instance).<\/span><\/li>\n<li><span style=\"font-size:11.5pt;\">The name and address of the executor or person handling \/ administering the estate.<\/span><\/li>\n<li><span style=\"font-size:11.5pt;\">This would include the name and address of the spouse if applicable.<\/span><\/li>\n<li><span style=\"font-size:11.5pt;\">Death certificate.<\/span><\/li>\n<li><span style=\"font-size:11.5pt;\">Marriage certificate if applicable.<\/span><\/li>\n<li><span style=\"color:windowtext;\">Birth certificate of surviving spouse if applicable.<\/span><\/li>\n<li><span style=\"color:windowtext;font-size:11.5pt;\">Birth certificate(s) of dependent children (under age 18 or 18 to 25 if still in school and\/or eligible for a survivor benefit).\u00a0<\/span><\/li>\n<\/ul>\n<p><em><span style=\"font-size:11.5pt;line-height:107%;\"><strong>Note: Include the deceased member\u2019s pension number on all documents.<\/strong><\/span><\/em><br \/>\u00a0<\/p>\n<h3>Other pension, financial and insurance benefits<\/h3>\n<p class=\"Default\"><strong>The Canada Pension Plan\u00a0<\/strong><\/p>\n<p class=\"Default\"><span style=\"font-size:11.5pt;\">CPP\u00a0<\/span>will pay up to two thousand-five hundred <strong>($2,500.00)\u00a0<\/strong>dollars towards the cost of the burial. This is a lump sum death benefit, and <em><strong>it is taxable<\/strong>.\u00a0<\/em>The payment is based on the total of the deceased\u2019s pension for the previous six months, i.e. six times the amount of the monthly pension payment but not exceeding <strong>$2,500.00<\/strong>. The spouse, estate executor or executrix <strong>must apply\u00a0<\/strong>for payment. Necessary forms are available at a Service Canada or the Canada Pension Plan office by calling <strong>1-800-277-9914.\u00a0<\/strong>Generally, the funeral home director will have these forms and others available as part of the funeral services.\u00a0<\/p>\n<p class=\"Default\">The Canada Pension Plan will also pay the surviving spouse up to sixty <strong>(60%)\u00a0<\/strong>percent of the deceased\u2019s monthly pension, the actual amount being dependent on the CPP being received by the spouse.\u00a0<\/p>\n<p class=\"Default\"><strong>Note:\u00a0<\/strong>If you and your spouse have a \u201csplit\u201d CPP, the payment is <strong>60%\u00a0<\/strong>based on the pensioner\u2019s \u201cfull\u201d entitlement. For the spouse to obtain this payment, he\/she must submit the appropriate forms. CPP payments are indexed to the Consumer Price Index and adjusted on January 1st each year.\u00a0<\/p>\n<p><a href=\"https:\/\/www.canada.ca\/en\/services\/benefits\/publicpensions\/cpp.html\">Visit the Canada.ca website for details.<\/a><br \/>\u00a0<\/p>\n<p><strong>Old Age Security (OAS)<\/strong><\/p>\n<p>Persons <strong>65 or older\u00a0<\/strong>are eligible to receive Old Age Security (OAS)\u00a0upon making application if they have <strong>resided in Canada for at least 40 years after age 18<\/strong>. People who have resided in Canada <strong>between 10 and 40 years\u00a0<\/strong>are eligible to receive a partial benefit. OAS benefits paid in a year are based on a person\u2019s income as reported on their prior year\u2019s income tax return.\u00a0<\/p>\n<p>OAS payments are indexed to the Consumer Price Index and are adjusted on a Quarterly basis (i.e. 1st day of January, April, July and October).\u00a0<\/p>\n<p><span style=\"font-size:12.0pt;\">The Income Security Programs, through Service Canada, <strong>must\u00a0<\/strong>be informed of your spouse\u2019s death for OAS purposes\u00a0<\/span><\/p>\n<p><span style=\"font-size:12.0pt;\"><strong>Required Information\u00a0<\/strong><\/span><\/p>\n<ul>\n<li>The full name and the Social Security Number (SIN#)<\/li>\n<li>The date of birth and the date of death<\/li>\n<li>The pervious address<\/li>\n<li>The name and address of the person responsible for handling the deceased\u2019s affairs (or next of kin)<\/li>\n<li>T4A\u2019s (income tax information slip) will be sent to the executor or person administering the estate\u00a0<\/li>\n<\/ul>\n<p>Old Age Pension has no survivor\u2019s benefits. This pension dies with the pensioner. There are, however, provision made for the following cases based on a <strong>\u201cmeans\u201d<\/strong>\u00a0test.\u00a0<\/p>\n<p><a href=\"https:\/\/www.canada.ca\/en\/services\/benefits\/publicpensions\/cpp\/old-age-security.html%C2%A0%20\">Web site<\/a><\/p>\n<p><strong>Guarenteed Income Supplement\u00a0<\/strong>(<strong>GIS) for old age pensioners:\u00a0<\/strong><\/p>\n<p><span style=\"color:black;font-size:11.5pt;\">This is an allowance for 60 to 64-year-old partners of <strong>GIC\u00a0<\/strong>recipients. It includes a <strong>Survivor Allowance\u00a0<\/strong>for 60 to 64-year-old widows. <strong>Note: If there is any doubt about the eligibility for this or any Federal pension call: 1- 800- 277-9914.\u00a0<\/strong><\/span><\/p>\n<p class=\"Default\">To qualify for this entitlement (GIS), you must be receiving an Old Age Security Pension, be a resident in Canada, and your income must be below the qualifying level. You need to check with the Pension Office to see if you qualify. It you do, you need not re-apply for this benefit each year if you file your income tax return by April 30th. As mentioned, it depends on your income, and it is a <strong>tested\u00a0<\/strong>benefit based on the income of both spouses.\u00a0<\/p>\n<p><a href=\"https:\/\/www.canada.ca\/en\/services\/benefits\/publicpensions\/cpp\/old-age-security\/guaranteed-income-supplement.html\"><span style=\"font-size:12.0pt;line-height:107%;\">Visit Canada.ca for details.<\/span><\/a><br \/>\u00a0<\/p>\n<h3>Public Service Health Care Plan and dental insurance plans<\/h3>\n<p style=\"margin-bottom:0cm;\"><a href=\"https:\/\/pshcp.ca\/coverage\/survivor-benefits\/\"><span style=\"color:blue;font-size:12.0pt;line-height:107%;\">Public Service Health Care Plan | Survivor Benefits<\/span><\/a><\/p>\n<p style=\"margin-bottom:0cm;\">In most cases, your eligible dependents can continue to receive coverage under the Plan in the event of your death.<\/p>\n<p style=\"margin-bottom:0cm;\">Surviving dependents must complete a\u00a0<a href=\"https:\/\/www.pshcp.ca\/wp-content\/uploads\/2013\/07\/pens-applic-adhes-006492-2.pdf\" target=\"_blank\">Pensioner Application Form<\/a>, which they can obtain from the Compensation Advisor responsible for the file of the deceased Plan member, or from the pension office appropriate to their employer. The application must be submitted within 60 days of the Plan member\u2019s death so that coverage may commence the first day of the month following the month the application is received by the designated officer.
